AMCOS/APRA Forms: Performer's Declaration (Form 1), Adjudication Copy (Form 2) and Accompanist Copy (Form 5) will be sent by post to the entrant with the programme. The entrant must complete Form 1 only once for the duration of the Balaklava Eisteddfod and present it to the Booking Officer at each venue where performing. These forms can also be downloaded from APRA website under the Music Users Publications and Guides heading. There is a new publication called Music Copyright for Eisteddfods - This guide was prepared at the request of the National Association of Eisteddfod Societies for its members & competitors.

Music Copyright for Eisteddfods
(http://www.apra.com.au/music-users/downloads/eisteddfods_2006.pdf)
This publication provides copyright guidelines for eisteddfod organisations and competitors for music, speech, drama and dance competitions.
(This is a pdf document and the forms are on pages 12-14)

It is also recommended to read A Practical Guide to the Use of Print Music
This guide is for private music teachers, music students, music administrators, community choirs, orchestras, eisteddfodau, conductors and composers. This is an update of the publication we formerly referred to.
