Select and "click" on the desired form. When it appears (Windows/Word Pad document) you can either PRINT the form to complete later or type in the required information and then PRINT.

Field Trip Waiver:  This form needs to be submitted each semester by all band & colorguard students.  It is the MVUSD medical release form.

Band Expense Sheets:  This form is used for ordering Band t-shirts, pep outfits, and booster membership.

Guard Expense Sheets: This form is used for Color Guard camp, equipment, and booster membership.

Scrip:  This form is used for weekly scrip orders.  Payment must accompany this form.

Practice Chart:  This form is to record your weekly practice time outside of class.  It must be signed by a parent/guardian.
