          Adoption Agreement

This form is to help make sure that certain criteria are met after the adoption.

1. If for any reason the dog/puppy cannot remain with you, it will be returned to   Great Dane Rescue of Northern California, Inc. immediately. Please contact us if you cannot keep the dog/puppy. If you move, you will notify  us of your new address and phone number at least two weeks prior to moving, so that we may keep in touch with you.

2. You will provide for yearly vaccines, heartworm preventative (daily or monthly), flea and tick preventive (if needed) and all other general care to keep the dog/puppy in good health. (We may ask for proof to verify that these things are being provided)

3. Contact us immediately if the dog/puppy is missing. (ran away or taken)

4. You will provide adequate food, water, and the dog/puppy will be kept as an indoor pet. You must have an adequately fenced yard or area for the Dane.

6. The dog/puppy will be spayed or neutered.

7. You must agree to post-adoption follow-up. If any of these conditions are not being met, the dog/puppy may be taken back immediately.
