      MUSIC CO. and FSO ENTERTAINMENT GROUP, we will need to verify certain information

      and references you provide, in order to establish a ministerial relationship with you or your

      group. Also, if you provide information, photographs, or other media for use on the Official

      Krusade Website, we will need your name to give you credit. If you, as a submitter, wish to

      remain anonymous, please indicate your preference when submitting your information, and

      we will honor your request.

 

(2) YOUR ADDRESS - We will need this information, also, if you wish to receive materials from

      us that we send via the USPS.

 

(3) YOUR TELEPHONE NUMBER - While this information is rarely needed, sometimes email

      addresses or even physical addresses become outdated. We request (but do not require)

      your telephone number when joining our USPS mailing list so we may be able to contact

      you in the event other methods of communication fail. For your privacy, we encourage you

      to guard your personal and home phone number from anyone you would not want calling

      you or locating where you live. When and if you provide such information to us, we will
