M.I.S.O. Forms

M.I.S.O. forms are slowly being added to this website.  Click on our Documents link at:  http://groups.msn.com/MISO/files.msnw?fc_p=%2FMISO%20Forms&fc_a=0  to download your forms.These forms can not be filled in on the website, nor can they be e-mailed back to the club.  The forms currently on the site can be accessed only by members of this community.

There are two methods for obtaining the forms.  First, you can go to the menu on the left column of the main page, click on Documents then click on MISO Forms.  Right-click the form you want, then select Print Target from the drop-down menu.  The form will print out on your printer.

The second method requires that you have MicroSoft Office.  Click on Documents then click on MISO Forms.  Left-click the form you want and it will automatically bring up the appropriate program and the form will open in that program for you.

Forms that need to be submitted to M.I.S.O. may be presented to any club officer or mailed to:

You will receive a reply by e-mail when your completed form is received.  The forms currently on the website are the M.I.S.O. Application Form to apply for club membership and the Incident Report Form to report a violation of safety rules, M.I.S.O. rules or some other serious issue involving club members that should be addressed by the club officers.

Application Form:  To use this form, download and print it out.  Xerox the form with a valid A.M.A. Membership Card ("Pilot's License") for the calendar year for which you are applying.  Fill in the appropriate spaces after the A.M.A. card is copied onto the form.  Sign and submit the form.  Cash will be accepted if the form is submitted in person to a club officer, do not send cash in the mail.  If mailing this form, include a personal check made out to cash.  Make a note in the comments section of the check indicating that it is for membership to the club and also the year for which it is being submitted.  Please note that membership to M.I.S.O. can not be accepted without proof of A.M.A. membership.

Your membership approval will be sent to you by mail.  If your membership is not accepted, your check or cash will be returned to you along with an explanation of why your membership could not be approved, i.e. "No valid A.M.A. Membership".

Incident Report Form:  Use this form to report unsafe, unsportsmanlike conduct, or other incident that you witnessed and which requires action by the club.  Do not use this form to report problems with radios, trees, field conditions, obstructions, etc.  This is only to report that a member violated a M.I.S.O. rule or the A.M.A. Safety Code.

Print out the form as noted above, print clearly, fill in all relevant information including the names of witnesses.  If the witnesses are M.I.S.O. or A.M.A. members, indicate by circling where appropriate.  Submit in person to any M.I.S.O. officer or mail to the address above.  A copy of your complaint will be mailed to you after the incident is investigated and you will be informed at that time of the final disposition.

Contest Score Sheets:  Blank copies of the score sheets for the M.I.S.O. Glider Golf, Duration Fun-Fly, Sloper Fun-Fly and Soaring Safari are now available on this site.  Forms will be available to contestants at all contests.  Anyone who may wish to practice the various tasks associated with any particular contest may wish to print-out a form and use it to during practice sessions.
