..1-800-824-4546 Face-To-Face is a project of the Educational and Research for the American Academy of Facial Plastic and Reconstructive Surgery (AAFPRS) in cooperation with NCADV. The purpose of the program is to assist survivors of domestic violence who have suffered facial, head and neck injuries as a result of a domestic assault by an intimate partner or spouse. ..1-800-773-GBAS(4227) The GBAS Program is a project of the National Humanitarian Project of the American Academy of Cosmetic Dentistry (AACD) and the AACD Charitable Foundation in cooperation with NCADV. The purpose of the program is to assist survivors of domestic violence who have suffered injuries to there front teeth and/or supporting structures of the front teeth as a result of a domestic assault be an intimate partner or spouse. AACD cosmetic dentists offer complimentary construction and cosmetic and reconstructive dental care to qualifying applicants. SKIN CARE OUT REACH EMPOWERS SURVIVORS (S.C.O.R.E.S.)
. 1-888-892-6702 The SCORES Program is a project of the American for Dermatologic Surgery (ASDS) in cooperation with NCADV. The purpose of the program is to assist survivors of domestic violence who have suffered skin injuries (scares, tattoos, burns) to their torso as a result of a domestic assault by inmate partner or spouse. ASDS dermatologic surgeons offer complimentary consultation and dermatologic surgery to qualifying applicants. An NCADV advocate walks potential Applicants through the initial screening and application process and provides liaison between the public and AAFPRS, AACD, and ASDS. The above national tool fee numbers have been designated for individuals or local agencies to call in order to receive information or because involved in Face to Face, GBAS or the SCORES Program. Women and men who have sustained injuries as a result of a domestic assault from a partner or spouse and are out of the abusive relationship qualify for services. Survivors of domestic violence can begin to heal physically and emotionally as the scars of the violence are repaired and removed. Procedures For Those Individuals Wishing To Apply -
Call the above-listed toll free number and leave a name, area code and phone number. -
Calls will be returned as soon as possible, and a telephone screening interview will be conducted. -
A referral to a local domestic violence agency will be given, if necessary. -
If the caller qualifies, an application will be sent to them in the mail. -
Fill out and sign the front of the application and make an appointment with a counseling or advocate agency. -
Have a counselor or advocate fill out and sign the back of the application and return the from to the Applicant. -
Mail (DO NOT FAX) the completed application to NCADV. -
When NCADV receives the application, all forms will be forwarded to AAFPRS, AACD or ASDS. -
AAFPRS? AACD? ASDS will make ever effort to refer a participating physician in the Applicants area. Please note that the Applicant must meet at least once with a counselor, therapist, social worker or domestic violence advocate in order to complete the application process. An in-person meeting is necessary to determine if the injuries are a result of domestic violence, whether the applicant is out of the abusive situation and whether the Applicant is a good candidate for the program. This requirement also serves to connect Applicants with their local support programs. Feel free to copy and circulate this letter and any related materials.
